How can I schedule an appointment? Contact the office at (586) 563-3300 or email us at appointments@ceasepainnow.com to request an appointment. The office is open Monday through Thursday 9:00 a.m. to 5:00 p.m. and Friday 9:00 a.m. to 4:00 p.m.
What do you need to bring to your first appointment?
Forms: Please feel free to click on the links below to open the required forms for our office. You may then print, complete, and bring these to your appointment with you. You will need Adobe Acrobat to view these files. If you do not have this installed on your computer, click here to download this program. Demographic - Patient personal and insurance information. Signature of File - Agreement between the patient and the physician stating that if the insurance company does not pay for services, the patient will be responsible for the balance. Copay and deductible amounts will be reflected on this form once insurance benefits have been verified. Office Policies - List of office policies related to insurance, payment, appointments, etc. HIPAA Notice of Privacy Practices - This form advises the patient of their rights to privacy, how we may use their personal information, disclosure of personal information, etc.
What should you expect at your first appointment? You initial visit will consist of a comprehensive physical examination and discussion of your health history. A personalized treatment plan will then be developed with you. No procedures (i.e. acupuncture, injections, etc.) will be performed at your first visit. Dr. Wilson will continue to monitor your progress on a monthly basis, or more frequently if required. All findings will be forwarded to your primary care physician.
Insurance Plans Accepted:
* We DO NOT participate with PPOM. You may have out of network benefits that would cover a portion of the services covered. You would then be responsible for a higher out of pocket copay or deductible amount. Please click here for a map to our office. Parking is in the back of the building.
